1. Accident and Incident Reporting

Accident and Incident Reporting

When accidents occur on Company premises the following procedures must be followed in order that the Company complies with the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR). All accidents to employees, guests or others coming onto our premises (this includes the land surrounding the building which is owned by Mitchells & Butlers) must be reported using the Electronic Incident Report Form which is sent automatically to the Safety Assurance Team. You should follow the Accident Reporting Procedures found in section 3.17 Incident Reporting and Accident Investigation of the Safety Policy. The Safety Assurance Team have to make any required RIDDOR reports. The cause of all accidents must be investigated by the Manager and remedial action taken where necessary and recorded on the Hazard Follow Up Form. Such accidents include: Any member of the public being taken from our premises to hospital as a result of an accident. An accident to an employee which falls into any of the following categories:

Any accident which results in a fatality A fracture of a major bone (not hand or foot) A fracture of the skull, spine or pelvis An amputation of a hand, finger, thumb, foot or toe

The loss of consciousness

The loss of sight

Any injury resulting in hospitalisation for more than 24 hours

A Dangerous Occurrence which includes: •

Collapse or failure of a hoist or lift

Explosion or collapse of any closed vessel e.g. Boiler Electrical short circuit or overload causing fire or explosion

Explosion or fire causing 24 hour closure

Uncontrolled release of large quantity of flammable gas or liquid

Generic Checklist: If a serious accident occurs on your premises the following procedures should be followed: Ensure that the area is safe and that further people cannot be injured. This may involve closing off an area or piece of equipment Obtain full details of the incident and of any third parties involved, taking names and addresses of witnesses Where outside officials are involved, always check their identity making a note of their name and Authority and if possible obtain a business card Do not make any comment to the press but instead refer them to Public Relations at Retail Support Centre (RSC) Ensure that you complete the Incident Report Form as soon as practically possible after the event and before you go off duty Contact your RBM or if he/she is not available, your RD and inform him/her of the details In the event of a serious emergency you should contact the Safety Assurance Team or Fleet Street Security if Out Of Hours on: 0121 4985960 If a serious accident occurs on your premises involving a hotel guest the following procedures should be followed additionally. • • • • • • •

Lodge Specific:

If required, put the room out of order on Opera (including the reason why) and notify Central Reservations

Notify other guests who may be travelling with the injured guest Collect all reservation documentation relating to the injured guest

If guest travelling alone, contact home address to notify family member of the accident. Take advise via your RBM if he/she is not available, your RD and inform him/ her of the details

Provide situation updates to other guest when required
